Job Summary

Home Health & Hospice Nurse, Home Health Hospice Located in an area serving home health and hospice patients, this role involves providing comprehensive care to patients in their homes. The position requires recent home health experience, proficiency in Outcomes and Assessment Information Set (OASIS) documentation, and the ability to manage a variety of clinical needs including wound care and IV therapy.

Job Details

• Certifications and Qualifications: BLS (AHA); Local rate based on candidates living within 80 miles of the facility; recent home health experience (within 2 years), proficiency in OASIS, wound care expertise, and excellent infection control practices in the home environment; must hold a valid license to submit.
• Schedule: Rotating shifts, Monday to Friday from 0800 to 1630, with rotating weekends, holidays, and on-call responsibilities, totaling 40 hours per week.
• Responsibilities: Providing skilled nursing care to home health and hospice patients, including SOCs, re-certifications, and resumption visits; managing a typical caseload of approximately 6 patients per day; participating in rotating on-call duties, with primary hospice patients generally not assigned but may be seen on call.
• Additional Information: The role involves coordination within the office scheduling system, adherence to dress code of any solid, clean, and professional attire, and clocking in/out at the facility. Prior experience with Marshfield must be noted, with a minimum six-month gap from previous employment.
