Job Summary
Home Healthcare Nurse in the Home Healthcare (HH) specialty is responsible for performing comprehensive nursing assessments upon patient admission and developing personalized care plans. This role involves evaluating clinical needs, coordinating discharge processes, and ensuring the safety and quality of patient care within a community setting.

Job Details

• Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Current Registered Nurse (RN) license in Massachusetts with at least 1 year of nursing experience; CPR certification required; home care and IV experience, computer skills, and a Bachelor of Science in Nursing (BSN) are preferred.
• Schedule: 8-hour shift, Days, 40 hours per week.
• Responsibilities include conducting thorough nursing assessments, maintaining documentation, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.
• Participates in patient case conferences, updates care plans as necessary, and ensures all required paperwork is completed accurately and in a timely manner.
• Instructs and supervises the Home Health Aide every 14 days, and refers to community resources as appropriate.
• Initiates and revises patient care plans, addresses all problems identified, and documents clinical data and interventions clearly.
• Coordinates discharge planning and communicates changes in patient condition to physicians and care team members.
• Completes all admission, progress, and discharge paperwork.
