Job Summary
Hospice RN Case Manager located in Montana. This role involves providing skilled nursing, care, and case management for patients in their residences. The position supports continuity of care through coordination, assessment, planning, and patient/family instruction within an interdisciplinary team approach, with potential transition from Visiting RN to Case Manager.
Job Details
- Required Qualifications: Current RN license in the State of Montana (BSN preferred), valid drivers license, appropriate auto insurance, and reliable transportation. Two years of recent acute care, home care, or hospice experience including clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred. HCHB experience is strongly preferred.
- Schedule: Day shift, 8 hours per day, minimum 40 hours per week.
- Responsibilities include delivering skilled nursing and coordinating care in a residential setting through a team approach, evaluating patients' physical and psychosocial statuses, overseeing care provided by Home Health Aides and CNAs, maintaining accurate patient records, participating in team scheduling and on-call rotations, and engaging in quality improvement activities.
- The position involves assessing and evaluating patient needs via telephone and home visits as required and providing education, counseling, and referrals to promote health and quality of life.
