Home Healthcare Registered Nurse (RN) West Portland, Oregon and Surrounding Areas. This position involves providing skilled home health nursing services to adult and geriatric patients within their homes. The role requires direct patient care, assessment, and management, ensuring safe and effective delivery of services in a community setting.
Job Details
- Position Description: Backfill for a Home Health RN in the West Portland area requiring Oregon licensure. Shifts are scheduled for 8 hours during daytime hours, with a minimum of 40 hours per week.
- Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: BCLS certification, Oregon State Licensure, 1 year of skilled home health experience, and valid Oregon or Compact license in hand.
- Schedule: Days, 8-hour shifts, minimum 40 hours per week.
- Additional Information: The RN is expected to visit up to 6 patients per day, with a focus on safe and effective patient assessments and care plans. Travel is within a roughly 60-mile radius, utilizing efficient grouping of clients to minimize drive time. The role includes charting on EPIC and Rover systems, with no on-call requirement. Candidates must have own transportation and be willing to work in a community setting, adhering to required safety and clinical protocols.
- Submission requirements include two professional references covering the last three years and confirmation of prior experience with a Providence facility if applicable.
