Job Summary
Hospice Registered Nurse (RN) Location unspecified. This role involves coordinating and delivering comprehensive end-of-life care to terminally ill patients in accordance with hospice philosophy and regulatory guidelines. The position requires managing a caseload of patients, conducting thorough assessments, developing individualized care plans, and providing skilled nursing interventions to address physical, emotional, and psychosocial needs. The nurse serves as the primary point of contact for patients, families, and the interdisciplinary hospice team, ensuring seamless communication and continuity of care.
Job Details
- Education and Work Experience: Bachelor's Degree in Nursing (BSN) preferred; five years hospice nursing experience preferred; registered nurse licensure in the state of practice required; CPR or BLS certification from approved vendor required; valid drivers license and at least 21 years of age required; current automobile insurance and reliable automobile required.
- Schedule: Rotating shift, 8 hours per shift, minimum of 40 hours per week.
- Essential Functions: Conducts initial and ongoing assessments of patients physical, emotional, psychosocial, and spiritual needs; develops and updates individualized plans of care in coordination with physicians and the interdisciplinary team. Delivers skilled nursing care including medication management, pain and symptom control, wound care, and patient and family education. Provides emotional support and crisis intervention as needed. Serves as the primary case manager for an assigned caseload (typically 1218 patients), ensuring continuity of care. Communicates regularly with team members, participates in interdisciplinary team meetings, and ensures compliance with all regulatory documentation and requirements. Responds promptly during on-call responsibilities and performs other related duties as assigned.
