Job Summary
Hospice RN Hospice Cape Cod, MA. This position involves providing skilled nursing care, supportive care, and pain and symptom management to maximize comfort and quality of life for hospice patients and their families. The role includes travel to patients' homes across the entire Cape, including the bridge, Yarmouth, Harwich, and Orleans areas, in a patient-centered, interdisciplinary team environment.

Position: Hospice RN

Schedule: Days shift, 8-hour shifts, minimum 40 hours per week
Required Qualifications: Current RN license in Massachusetts; 2 years of Hospice RN Case Management experience; IV Certification, Computer skills, and BSN preferred.
Preferred Qualifications: Hospice Home Care experience; additional certifications such as IV Certification.
Additional Information: The Hospice RN provides care in both private homes and healthcare facilities, participating in interdisciplinary patient case conferences, assessing patients critically, managing time efficiently, and communicating effectively with patients, families, and healthcare teams. Responsibilities include documenting patient care clearly, implementing physician-directed plans, and supervising Hospice Aides. The role requires adherence to standards set by the American Nurses Association and understanding of hospice philosophy. Supervises Hospice Aides every 14 days and assists in orientation of new staff when requested. The successful candidate must pass a pre-hire medication test and complete admission paperwork by 9am the next business day.
