Home Healthcare RN (Registered Nurse) in Wausau, Wisconsin. This position involves providing home health care services to patients outside a 50-mile radius from the facility, with a focus on managing patient care through visits, assessments, and documentation. The role requires a minimum of two years of home health experience and active licensure.

Job Details

• Position Description: Home healthcare nurse responsible for managing patient care in the home setting, including Oasis admissions, all time points, episode management, doctor calls, and care coordination. Documentation must be completed in the electronic medical record in a timely manner. Clinical competencies include wound care, IV therapy, ports, lab draws, and chronic disease management.
• Qualifications: Active Wisconsin or Compact license, AHA or ARC Basic Life Support certification, valid drivers license, Oasis certification a plus, and a minimum of 2 years of home health experience. Prior travel experience is required.
• Schedule: Shift from 8:00 AM to 4:30 PM, approximately 8 hours per day, with a guaranteed 40 hours per week.
• Additional Information: The position involves travel between 25 to 60 miles daily depending on patient care territory, with mileage reimbursed at the government-approved rate. There may be potential to travel to Medford, Antigo, and Wisconsin Rapids. The role includes participation in weekend and holiday rotations, every fifth weekend and holiday. During flu season, declination of flu and COVID vaccinations requires wearing a mask at all times, per policy.
