Job Summary:
Long Term Care (LTC) Registered Nurse (RN) position in a facility providing short-term rehab, long-term care, and skilled nursing services. This role involves performing initial assessments, medication administration, overseeing Certified Nursing Assistants, skin and pain assessments, admissions, wound care, and pressure ulcer prevention. The position requires flexibility to rotate between shifts and on-call duties, supporting a team-oriented environment focused on patient-centered care.

Job Details:

• Certifications: BLS (Basic Life Support) certification required. No IV certification needed for Licensed Practical Nurses (LPNs).
• Education & Experience: Minimum of 2 years of relevant experience; accepting first-time travelers.
• Schedule: Rotating shifts with day (0600-1430), evening (1415-2245), and night (2230-0700) shifts. Can mandate staying 4 hours past shift for overtime per union rules and rotations between shifts to meet staffing needs.
• Clinical Responsibilities: Conducting assessments, medication pass, managing skin and pain evaluations, admissions, wound care, pressure ulcer prevention, and assisting staff as needed.
• Staffing & Operational Details: Staffing ratio of 1 RN to 23-28 residents; approximately 36 hours per week; 12-hour shifts; rotating weekends; may be asked to participate in holiday rotation.
• Facility & EMR: Capable of working with Meditech electronic medical record system, which will transition to a new system in June 2024.
• Team & Environment: Collaborates with physical and occupational therapists; participates in daily interdisciplinary team meetings.
• Additional Details: Responsible for dressing in any scrub color, completing department-specific orientation including initial training and preceptor shadowing.
• Licensing & Submission: Active license required for licensing submission.
