Job Summary:
Medical Surgical Skilled Nursing (MS SNF) in a skilled nursing facility setting. The position involves coordination of the Minimum Data Set (MDS) process, ensuring compliance with federal and state regulations. The MDS Coordinator collaborates with healthcare professionals, residents, and families to develop comprehensive care plans and facilitate interdisciplinary team meetings, supporting optimal patient care.

Job Details:

• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Required Qualifications: Must possess a Nursing Degree from an accredited college or university, an RN or LVN California License, current CPR certification, and one year of verifiable experience as a licensed nurse providing care and services in a skilled nursing or long-term care setting.
• Preferred Qualifications: MDS certification and current RN California License.
• Additional Information: RTO is not approved unless noted in the offer.
