Wound Care Nurse Wound Care Specialty [Location]
This position involves conducting comprehensive health assessments focused on wound, ostomy, and continence related care. The role includes developing personalized care plans, evaluating treatment responses, and collaborating with the healthcare team to ensure optimal patient outcomes.
Job Details
- Conducts health assessment interview focused on wound, ostomy, and continence related care. Performs focused physical assessment, develops plan of care based on the diagnosis and individual client needs. Recommends medications and treatments as appropriate.
- Develop, revise, and evaluate standards of care, assist in the selection of supplies, technology, and equipment.
- Assess and document patient response to wound care, ostomy, continence, fistula, and complex wounds treatment. Evaluate the quality and delivery of care provided by the clinical staff on the unit.
- Works collaboratively with the healthcare team and makes referrals to other disciplines as indicated.
- Teach, counsel, and assist individuals and families to assume responsibility for prevention of complications related to patients with ostomies, wounds, or continence problems.
- Develop, provide, and evaluate staff education programs, including new employee orientation, in-services, and continuing education programs.
- Routinely provide comprehensive assessments of patients at risk for developing pressure injuries and initiate preventative measures.
- Escalate and communicate challenges identified, including ethical issues and conflict resolution.
- Collaborate with the assistant director of nursing for wound care to initiate and recommend best practices based on prudent assessment.
- Participate in unit/department quality improvement and hospital-wide quality improvement activities, such as incidence and prevalence studies.
- Provide culturally competent patient, family, and staff education respecting various belief systems.
- Maintain the security and confidentiality of data and information in accordance with patient rights policies.
- Achieve and maintain a professional level of competency through ongoing activities aligned with clinical practice and patient population.
- Identify community resources helpful to clients, refer, and assist clients in utilizing such resources.
- Asset management related to rented equipment, e.g., specialty beds and negative pressure devices.
Eligibility Requirements:
- Associates Degree (ASN, ADN) from an accredited nursing program.
- New York State RN License.
- Primary source verification of license and registration of registered professional nurse from NYS Department of Education (NYSED).
- AHA BLS certification.
- Current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN), Certified Wound Ostomy Nurse (CWON), Certified Wound Care Nurse (CWCN), Certified Wound Specialist (CWS), or Wound Care Certified Certification (WCC).
Schedule
Days shift 7.5 hours per shift minimum of 37.5 hours per week
