Job Summary
Wound Care Specialist Location: The organization is seeking a dedicated Wound Care Specialist responsible for conducting health assessment interviews focused on wound, ostomy, and continence-related care. This role involves performing focused physical assessments, developing individualized plans of care, and collaborating with the healthcare team to optimize patient outcomes.

Job Details

• Required Qualifications: Associate Degree (ASN, ADN) from an accredited Nursing Program; New York State RN License (verified by NYS Department of Education); AHA Basic Life Support (BLS); current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN), Certified Wound Ostomy Nurse (CWON), Certified Wound Care Nurse (CWCN), Certified Wound Specialist (CWS), or Wound Care Certified Certification (WCC). - Schedule: Day shift, 7.5 hours per shift, minimum of 37.5 hours per week. - Responsibilities include assessing and documenting patient responses to wound care, ostomy, and continence treatments, evaluating care quality, and making referrals as needed. - Collaborates with the healthcare team, initiates and recommends best practices based on assessments, and participates in quality improvement activities. - Provides patient and family education to prevent complications related to wounds, ostomies, or continence issues. - Conducts staff education programs, assists in asset management of rented equipment, and maintains patient data confidentiality. - Additional Skills: Health assessment interview, wound care, ostomy, continence-related care, focused physical assessment, quality improvement, staff education programs, asset management, plan of care, patient response documentation, pressure injuries. - Positions start approximately on 11/02/2026 and end on 02/06/2027.
