Job Summary

Registered Nurse, Long Term Care. This position provides skilled nursing care to adult and geriatric patients in a long-term care setting. Responsibilities include patient assessments, medication administration, care planning, treatment coordination, and collaboration with interdisciplinary support services.

Job Details

• Required Qualifications: Active Wisconsin Registered Nurse license or compact Registered Nurse license; Basic Life Support (BLS) certification from the American Heart Association (AHA); at least 1 year of long-term care experience; at least 2 years of acute care experience within the last 3 years in the submitted specialty.
• Preferred Qualifications: Previous charge nurse experience.
• Schedule: Day shifts, 8 hours per shift, 06:3015:00, 40 hours per week, with every other weekend rotation and some holiday coverage.
• Additional Information: The role supports adult and geriatric patients and includes admission, discharge, and transfer coordination; medication administration; patient assessments; wound care; diabetes management; catheter, feeding tube, tracheostomy, ostomy, and intravenous therapy; infection control; fall and pressure-ulcer prevention; patient and family education; and end-of-life care. Bedside shift reporting, AIDET utilization, and hourly rounding are required. Staffing includes 1 Registered Nurse per hall, medication technicians per hall, and certified nursing assistant ratios of 1:810 on days and 1:20 on nights. The night-shift Registered Nurse may assume charge duties and may be the only Registered Nurse in the facility. A pre-start cardiac rhythm interpretation examination is required, with strict passing criteria and up to two attempts.
