Position: Registered Nurse (RN) Long Term Care (LTC)
Location: Delano, California
Certification and Education: Registered Nurse (RN) licensure in the state of practice; Basic Life Support (BLS) certification from an approved vendor; ACLS certification from an approved vendor. A Bachelors Degree in Nursing (BSN) is preferred.
Experience: Minimum of 0 years required.
Schedule: Day shift, 12-hour shifts, minimum of 36 hours per week.
Essential Functions: Collects and analyzes patient data, develops and implements care plans, evaluates progress, and collaborates with patients, families, and healthcare teams to promote health and safety. Delegates tasks appropriately and participates in discharge planning.
Additional Information: The RN will directly provide health information to patients and their families, coordinate care, and ensure continuity of care through effective discharge planning. The role emphasizes safety, team collaboration, and patient-centered care.
