Position: Interim Staff Pharmacist
Facility Name and address: Old Vineyard Behavioral Health: 3637 Old Vineyard Rd, Winston Salem, NC 27104
Licensed Beds: 250
Average Daily Census: 195
Compensation: We need to keep our BR within $125 - $135/hr which allows us to pay as much as $90/hr
Shift: 4x10 schedule - 12:30p – 11:00p four days a week. No weekends. No on-call
Dates of Coverage needed: ASAP for 13 weeks with possibility of extension.
Site Automation: Omnicell Cabinets and Robot- can train
Site Specialty Areas: We have an ICU, a NICU, med surg floors, and an orthopedic hospital.
Staff Size: 18 pharmacists, 18 pharmacy technicians
How many days of training/orientation are required prior to start? ~ 7/8 onsite
Interview: We will conduct an initial phone screen with the HSD, Jack.
Qualifications:
Tech support: techs mix IVs, answer phone calls and manage the medication requests.
Dress Code: Green scrubs, or business casual with white coat

Qualifications:
- Florida pharmacist licensure
- 1 year inpatient pharmacist experience
- Trained in Epic EHR
- If they have Omnicell - please put it on resume but they will also train on this so not required
- Order verification, physical product check, drug information to physicians and nurses.

Dress Code: Green scrubs, or business casual with white coat
CPS credentials required prior to start:
- Criminal background

- DT
- OH Testing because i't's within a hospital - TB, flu, rubella, MMR, varicells, TDap, Hep B, Color vision test
- These are hospital pharmacists so they should be able to provide their own records and if they do, they will not have to go to the OH appt.
