Job Summary:
To provide treatment adherence education and Direct Observation Therapy (DOT) for HIV ART medication and as requested by MD, Hepatitis C medication to increase the goal of HIV and or Hepatitis C. viral suppression
Job Responsibilities:
-Provides patients with education on prescribed medication including potential side effects and documents all education in the patient's medical record
-Dispenses medication to the patient as per the medical providers' order (daily, weekly, or biweekly)
-Documents all DOT encounters with patient on the DOT log sheet.
-Documents all DOT encounters in accordance with policy.
-Reports and documents all patient-reported or witnessed side effects to a PCC medical provider the same day.
-Obtains medication orders from the medical provider.
-Communicates with the pharmacy regarding patient medication and acts as liaison between the pharmacy and the provider.
-Receives ordered ART medication from pharmacies and stores the medication in the locked storage cabinet at PCC
-Ensures DOT patients are following the plan of treatment and notifies provider regarding non-adherence with visits, labs, or medication. Documents communication with provider regarding adherence issues.
-Participates in multidisciplinary case conferences with the primary care staff
-Actively participates in QA and QI projects and interventions.
-Participates in HIV/AIDS forums, working groups and conferences to stay abreast of developments in the sector, expand networks, and give visibility to IMC work in the prevention and treatment of HIV in addition to continued growth in compliance in the testing laws for HIV.
-Regularly attends PCC staff meetings
-Ensures an updated list of DOT patients is always readily available
-Schedules patients for DOT sessions and outreaches to patients who miss DOT.
-Reschedules DOT appointments and documents outreach attempts for missed DOT appointments.
-Assists Primary Care Nurse with triaging patients and performing nursing duties in the clinic.
-Provides coverage for the Primary Care Center Nurse when not available due to scheduled days off and sick calls.
-Duties include performing vital signs, screenings, patient education, medication administration, POC testing, and other nursing duties required in the clinic.
-Ensures exam rooms are cleaned in the morning, between patients and at the end of shift.
-Ensures exam rooms are restocked.
-Ensures equipment in exam rooms are cleaned and functioning.
-Orders medical supplies for the clinic monthly and as needed.
-Assists in preparing the huddle sheet for the daily interdisciplinary team huddle for the next clinic day.
-Performs prior authorizations as needed.
