Responsibilities : 

• Evaluate patients with physical functioning disorders and develop individualized plans of care.

• Utilize standardized assessments and evidence-based practices to guide clinical decision-making.

• Administer therapeutic interventions including manual therapy, gait training, therapeutic exercise, modalities, assistive devices, and wound care as appropriate.

• Monitor patient progress and modify treatment plans based on clinical outcomes.

• Document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and clinical standards.

• Educate patients, families, caregivers, and healthcare team members regarding treatment plans and functional goals.

• Participate in interdisciplinary meetings, care conferences, utilization reviews, and rehabilitation meetings.

• Maintain accurate documentation of treatment minutes and patient records.

• Assist in obtaining necessary durable medical equipment (DME) and therapy-related supplies.

• Supervise and support Physical Therapist Assistants (PTAs), rehabilitation aides, and students as appropriate.

• Practice in accordance with APTA standards, state regulations, and professional ethics.

Requirements : 

• Minimum of one (1) year of Physical Therapist experience (required).

• Long-Term Care (LTC) or rehabilitation experience (required).

• Current Massachusetts Physical Therapist license (required).

• New graduates will be considered if they have completed a Level II clinical rotation (Level I observation alone does not meet the requirement).
