REGISTERED NURSE I Total number of positions needed: 1 1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M – F, 8am – 5pm? M—Fri; 8-5 3. Will this candidate be visiting patients in care facilities or only in patient’s homes? both private homes and care facilities 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? will consider perm hire 6. What region are you in? Home health or Hospice? Home Health 7. How many miles per week will the consultant travel? 30/40 a day? Weekly? 50 / day 10. What are the weekly units of productivity expectations? 25/30? 30 points 11. What EMR systems will the consultant be using: HomeCare HomeBase, Oasis, Outlook 12. Will there be any on call? If so, how much? 1-2 weekends/month and holiday rotation; 1 week every 2-5 weeks, rotation based on staffing. 13. Do you want the candidate to have home health experience? Yes
