Travel Palliative Care Case Manager job in Martinez, California: Start ASAP for 13 weeks, working Monday through Friday, 8-hour days. You will spend 3 days each week visiting patients in their homes, assisted living, or skilled nursing facilities, averaging 5 visits per day. The other 2 days are dedicated to phone triage, offering flexibility and revisiting patients as directed by the nurse practitioner. Charting is clinic-based, focusing on basic assessments and reporting SBAR to practitioners or physicians. Home health experience is not required, but flexibility and strong communication skills are essential. Martinez offers scenic outdoor recreation, historic sites like the John Muir National Historic Site, and vibrant downtown dining and shopping.
