POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization

MINIMUM REQUIRED QUALIFICATIONS – FL/compact RN license; BLS; 2 years case manager experience in an acute hospital setting; Acute hospital experience in specialty during the past year with no more than a 90-day break

PREFERRED QUALIFICATIONS – Meditech and Midas experience; HCA experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8a-5p and every other weekend

SYSTEMS – Meditech and Midas

START DATE – ASAP
