This Home Health RN position is based in Greensburg, Kentucky, a welcoming small town in south-central Kentucky that offers a relaxed pace of life, friendly neighbors, and a true sense of community. In this role, you will provide skilled nursing care to patients in their homes throughout Greensburg and surrounding rural areas. Home health offers a unique setting where you can practice with a high level of autonomy while still having the support of a collaborative team. Typical responsibilities include performing comprehensive patient assessments in the home environment, developing and updating individualized plans of care, administering medications and treatments as ordered, monitoring chronic conditions, and providing education to patients and caregivers on disease management, medications, and safety in the home. You will coordinate closely with physicians, therapists, social workers, and other team members to ensure that care is well organized and that patients have the resources they need to remain at home safely. You can expect to see a diverse adult and geriatric patient population with conditions such as post-surgical recovery, heart failure, COPD, diabetes, wound care needs, and rehabilitation after hospitalization. Patient visits are scheduled to allow for thorough assessments and meaningful education, rather than rushed encounters. Visit volumes and daily caseloads are structured to balance quality of care with manageable productivity expectations in the home health environment. RNs in this setting typically work day shifts with some flexibility to arrange visit routes across the Greensburg area and nearby communities. On-call or weekend rotations may be required depending on agency needs, but the primary hours are during the daytime to align with patient and family schedules. The work is primarily field-based, traveling from home to home, with intermittent time spent on documentation and coordination using an electronic medical record system. You will join a supportive home health team that values collaboration, open communication, and shared problem-solving. Orientation and ongoing education are provided to ensure you are comfortable with home health regulations, documentation expectations, and specialized skills needed in the field.
