StartDate: 7/13/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – In an acute rehabilitation setting, coordinate and oversee each patient’s individualized care plan to ensure safe, efficient, and goal-directed recovery from admission through discharge.

MINIMUM REQUIRED QUALIFICATIONS –

• WA/compact RN license
• BLS
• 2+ years recent acute hospital case management experience
• Utilization Review experience
• Ability to support team and family conferences for acute rehabilitation (facility will assist/train to this IRF Team conference requirement)

PREFERRED QUALIFICATIONS –

• CCM, ACM, ARN, RN-BC-CM
• 2+ years recent acute rehab case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8a-4:30p M-F. EOW possible.

SYSTEMS – UDS PRO, Meditech, Pinpoint

START DATE – ASAP
