RN Care Coordinator
StartDate: ASAP Pay Rate: $1800.00 - $2700.00
POSITION SUMMARY RN Case Manager supporting colorectal surgery patients through pre-op and post-op care coordination.
POSITION DUTIESCoordinate pre-op and post-op patient care Schedule surgeries, imaging, and follow-up appointments Collaborate with providers and care teams Document care in Epic
MINIMUM REQUIRED QUALIFICATIONSActive RN license (MN required) 5+ years of RN experience Previous RN CM Experience Epic experience required
PREFERRED QUALIFICATIONSSurgical experience preferred Med-Surg background preferred Colorectal surgery experience a plus
LENGTH OF ASSIGNMENT 12–20 Weeks
SHIFT / HOURS PER WEEK Monday–Friday, Onsite
40 Hours per Week
