RN Case Manager – Hospice
StartDate: 8/24/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager – Hospice

POSITION DUTIES – Assesses patients, develops and coordinates individualized hospice care plans, manages symptoms and pain, provides nursing care and education, collaborates with the interdisciplinary team, and supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting.

MINIMUM REQUIRED QUALIFICATIONS –

• VT/compact RN license
• BLS
• 1+ year home health hospice case management experience
• Oasis experience

LENGTH OF ASSIGNMENT – 16 weeks

SHIFT / HOURS PER WEEK – Days, M-F with some weekend coverage

SYSTEMS – Oasis, Homecare Homebase HCHB

START DATE – ASAP
