In this role, you will provide skilled nursing care to patients in their homes across Portland and surrounding communities. You will conduct comprehensive assessments, develop individualized care plans, administer medications and treatments, and provide education to patients and caregivers. This position offers a high degree of autonomy, as you will independently organize your daily schedule, travel between visits, and manage a caseload in collaboration with the broader home health team. Your typical day may include conducting wound care, monitoring vital signs, managing IV therapies, reinforcing post-surgical instructions, and supporting patients with chronic conditions. You will be responsible for timely and accurate documentation in an electronic medical record system, coordinating with physicians and other disciplines, and participating in case conferences to ensure continuity of care. Patient ratios in home health are structured around visit schedules rather than traditional inpatient assignments, allowing for meaningful one-on-one time with each patient. You will work in varied home environments, ranging from urban apartments to single-family homes in surrounding neighborhoods. Shifts generally follow daytime hours, with some flexibility based on patient needs and agency scheduling practices. This role allows for a strong blend of direct patient care and clinical judgment, as you assess changes in condition, adjust care plans, and advocate for additional services when needed. The position is well-suited for nurses who value independence, relationship-based care, and community-focused practice.
