Registered Nurse – Home Health- Hospice - Travel - (HH RN)
StartDate: ASAP Available Shifts: 8 D Pay Rate: $2312.00 - $2455.00
This Home Hospice RN position is based in beautiful Orange County, California, a highly desirable region known for its sunny climate, scenic coastline, and vibrant communities. Orange County offers easy access to stunning beaches, outdoor recreation, and popular destinations such as Disneyland and Knott’s Berry Farm, as well as cultural venues, parks, and year-round events. With its mix of coastal, suburban, and urban neighborhoods, the area provides a welcoming environment, excellent schools, and a strong network of healthcare resources. As a Home Hospice RN, you will provide compassionate end-of-life care to patients in the comfort of their own homes throughout Orange County. You will work within a supportive, interdisciplinary hospice team that values collaboration, clinical excellence, and patient-centered care. Your role will focus on managing symptoms, enhancing quality of life, and supporting families during a challenging and meaningful time. Typical responsibilities include conducting initial and ongoing nursing assessments in the home; developing and updating individualized plans of care; coordinating with physicians and other team members to ensure appropriate medication and symptom management; educating patients and families about disease progression, comfort measures, and available support services; and providing direct hands-on care such as wound care, medication administration, and comfort-focused interventions. You will also participate in regular interdisciplinary team meetings to review patient status, contribute to care planning, and ensure continuity of care. Your day may include traveling between patient homes across an assigned region of Orange County, performing scheduled visits, and responding to urgent needs as appropriate. Visit volumes and patient caseloads are designed to allow for thorough, supportive interactions rather than rushed encounters, with an emphasis on holistic care that considers physical, emotional, and spiritual needs. You will have the opportunity to build meaningful relationships with patients and families and to see the impact of your care over time. Shifts typically follow daytime or early evening hours, with some flexibility based on patient needs and agency scheduling structures. There may be options for participation in on-call rotations to support hospice patients after hours.
