RN Case Manager – Hospice
StartDate: 9/8/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager – Hospice

POSITION DUTIES – Admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.

MINIMUM REQUIRED QUALIFICATIONS –

• CA RN license
• BLS
• 1+ year hospice case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:30am-5pm (Schedule can be flexible based on patient's availability). One weekend per month.

SYSTEMS – Homecare Homebase HCHB
