Key Responsibilities

• Provide physical therapy services to an adult and geriatric patient population in a home health setting
• Travel across a 4-county service area to conduct patient visits in the home environment
• Maintain a caseload ratio of approximately 1:5
• Document patient care using Home Care Home Base (HCHB), with a transition to Epic expected around September 2026
• Float within scope of practice as needed
• Adhere to holiday coverage expectations as outlined by the facility; travelers may be expected to work up to two major holidays during their assignment period

Required Qualifications

• Active Georgia PT license or Compact PT license (verify member state eligibility prior to submission)
• BLS certification required
• Minimum of 1 year of experience preferred; candidates with 6+ months of experience may be considered
• Willingness and ability to travel across a 4-county coverage area
• All RTO requests must be finalized at time of submission; no RTO or schedule changes can be requested post-offer or during orientation week; maximum of 7 days RTO per contract
• Local candidates are welcome and accepted

Preferred Experience / Skills

• Prior experience with Home Care Home Base (HCHB) documentation system strongly preferred; candidates without HCHB experience will be required to complete an online HCHB course prior to starting
• Experience working in a home health or community-based therapy setting
• Familiarity with Epic EMR is a plus given the anticipated system transition
