Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with interdisciplinary team members to support optimal patient outcomes
• Complete all required facility-specific documentation and credentialing steps prior to assignment start

Required Qualifications

• Active Physical Therapist license in the state of Oregon (compact license accepted at start, but permanent Oregon state license must be obtained promptly)
• Experience in an outpatient physical therapy setting
• Ability to fulfill contract QA credentialing requirements prior to start
• Candidates who have worked at this facility in a permanent or PRN capacity within the past year are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy experience
• Strong documentation and time management skills in a high-volume outpatient environment
