Key Responsibilities

• Conduct comprehensive patient evaluations (60-minute eval and treatment sessions) and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
• Promote patient and employee safety across key areas including safe patient handling, infectious disease protocols, and fall prevention within a defined circle of safety

Required Qualifications

• Active Physical Therapist license in good standing in the state of Florida
• Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
• Commitment to professional standards and applicable codes of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring physical therapy students
• Active involvement in continuing education and professional development activities
