Key Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with the patient, family, and interdisciplinary team
• Provide accurate, succinct, and timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy
• Participate in the patient rehabilitation program, utilizing the nursing process to assess, plan, implement, and evaluate patient care with a focus on patient and family education
• Initiate discharge planning from the day of admission and coordinate patient teaching in anticipation of the discharge date
• Communicate pertinent changes in patient status to attending physicians, charge nurses, and other care providers to support continuity of care and patient safety
• Provide direction, training, and supervision of Licensed Practical Nurses and Certified Nursing Assistants, delegating appropriately and utilizing effective problem-solving techniques

Required Qualifications

• Active Registered Nurse (RN) license valid in the state of Tennessee
• Demonstrated competency in nursing assessment, care planning, and clinical documentation
• Ability to work night shift in an inpatient rehabilitation setting
• Knowledge of safe patient handling, infection control, and workplace safety practices

Preferred Experience / Skills

• Prior experience in an acute rehabilitation or inpatient hospital setting
• Experience supervising or directing ancillary nursing staff such as LPNs and CNAs
• Familiarity with interdisciplinary team conferences and rehabilitation-focused care models
