Key Responsibilities

• Evaluate and treat patients in a home health setting across the assigned coverage area
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Monitor and document patient progress, adjusting plans of care as clinically appropriate
• Collaborate with interdisciplinary care teams to coordinate patient care and ensure continuity of services
• Educate patients and caregivers on therapeutic exercises, safety, and home management strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas (or eligibility to obtain)
• Graduate of an accredited Physical Therapy program
• Valid driver's license and reliable transportation for home health visits
• Ability to work independently in a home health environment

Preferred Experience / Skills

• Prior experience in a home health setting is a plus, though new graduates are welcome to apply
• Strong organizational and time-management skills for managing a home-based caseload
• Familiarity with home health documentation and electronic medical records
