Key Responsibilities

• Provide skilled physical therapy services to home health patients in accordance with the plan of care
• Complete documentation and charting accurately and in a timely manner using Wellsky
• Adhere to a point-based visit system, with 32–34 points expected per week
• Contact the appropriate party to report all missed visits or changes in evaluation frequency, or when unable to reach a patient
• Drive to patient locations; drive time is factored into scheduling, and overtime may be available if drive time pushes the workweek beyond 40 hours
• Maintain a clean and professional appearance, including scrubs and closed-toe shoes; facial jewelry limited to small nose stud and earrings

Required Qualifications

• Active Physical Therapist (PT) license
• Minimum of 2 years of recent licensed PT experience required (private duty experience does not qualify; new graduates are not eligible)
• Home health experience required; this may be a first travel assignment provided home health experience is met
• BLS and First Aid certifications required
• Proficiency or ability to learn Wellsky for clinical documentation

Preferred Experience / Skills

• Prior travel therapy assignment experience
• Familiarity with home health point-based productivity systems
• Strong communication and time-management skills in a field-based setting
