Key Responsibilities

• Provide outpatient physical therapy services to patients, with approximately 20% of visits conducted at assisted living facilities
• Attend daily stand-up meetings at 8:30 AM, Monday through Friday
• Manage a patient caseload ratio of 6–7 patients
• Document patient care and progress using Wellsky charting software
• Expected weekly mileage of approximately 50–75 miles for patient visits (may vary)
• Maintain clean, professional scrubs and closed-toe shoes per dress code requirements

Required Qualifications

• Active Physical Therapist license in good standing
• Minimum of 2 years of recent licensed PT experience; no new graduates
• At least 1 recently completed travel assignment required
• Experience with home care and/or HomeCare HomeBase (HCHB) required
• BLS and First Aid certifications required
• COVID-19 vaccination and booster(s) required per state guidelines; medical/religious exemptions considered on a case-by-case basis and may require weekly testing
• Must live at least 50 miles from the facility to be eligible

Preferred Experience / Skills

• Prior experience in outpatient and assisted living facility settings
• Familiarity with Wellsky electronic medical records platform
• Comfort with regular travel between patient locations
