Key Responsibilities

• Provide occupational therapy evaluations and treatments to patients in a skilled nursing facility setting
• Develop and implement individualized treatment plans to maximize patient functional independence
• Document patient progress and maintain accurate clinical records in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Must be open to floating between worksites as needed

Required Qualifications

• Active California Occupational Therapist license required
• Minimum 1 year of SNF experience required
• Standard credentialing requirements must be met prior to start

Preferred Experience / Skills

• Experience with Casamba documentation software highly preferred
• Familiarity with skilled nursing facility workflows and productivity standards
