Key Responsibilities

• Provide occupational therapy services to patients within a home health setting across an assigned territory
• Conduct evaluations and deliver treatment plans tailored to each patient's individual needs
• Meet expected productivity standards of 30 points per week (approximately 6 points per day)
• Document patient care accurately and in a timely manner using HCHB (HomeCare HomeBase) software
• Travel within a 50-mile radius of the office to provide patient care

Required Qualifications

• Active and unrestricted Occupational Therapist (OT) license in the state of Montana
• Previous home health (HH) experience required
• HCHB (HomeCare HomeBase) experience required
• Ability to work Monday through Friday, 8:00 AM – 5:00 PM; no weekend or on-call requirements
• Reliable transportation and ability to travel within a 50-mile radius of the office

Preferred Experience / Skills

• Experience managing a caseload independently in a home health environment
• Strong documentation and time management skills in a field-based setting
