Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction, utilizing 60-minute evaluation and treatment sessions
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities within an established length of stay
• Delegate and direct discipline-specific therapy services for assigned patients while maintaining accountability for clinical outcomes
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Adhere to all safety protocols including safe patient handling, infection control, and workplace hazard identification and reporting

Required Qualifications

• Active Physical Therapist license in good standing, compliant with the applicable State Practice Act
• Completion of all state-mandated continuing education requirements
• Demonstrated competency in clinical reasoning, treatment planning, and patient goal development
• Ability to ensure care is provided under signed, valid physician orders and within insurance authorization requirements
• Commitment to ethical practice in accordance with discipline-specific professional codes of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience with performance improvement initiatives and quality-focused care delivery
• Active involvement in professional organizations or ongoing continuing education beyond minimum requirements
