Contract | Days | Weekend rotations expected approximately once every 6 weeks

Key Responsibilities

• Assess patient conditions and develop, implement, and monitor individualized care plans in accordance with organizational philosophy, policies, and standards of community health practice
• Provide ongoing education and support to patients and their families throughout the course of care
• Perform admission processes and fulfill case management responsibilities in compliance with established policies and standards of practice
• Maintain thorough and timely electronic documentation and facilitate interdisciplinary communications
• Travel to assigned patient cases within the designated service area as required

Required Qualifications

• Active Physical Therapist license in the state of Massachusetts (or eligibility)
• Minimum 1 year of experience within a Certified Home Health Agency — experience in hospice, private care, or case management outside of a certified home health setting does not fulfill this requirement
• Minimum 1 year of case management experience
• Ability to travel to patient cases as assigned

Preferred Experience / Skills

• Familiarity with electronic medical records (EMR) and point-of-care documentation systems
• Strong interdisciplinary communication and collaboration skills
• Experience functioning in a float or multi-site capacity
