Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs and physician direction.
• Collaborate with the interdisciplinary team to address barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and coordination of continuing services.
• Manage individual, concurrent, and group treatment sessions effectively, demonstrating flexibility in caseload management and ensuring continuity of care throughout the length of stay.
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with applicable state, federal, and intermediary guidelines.
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Adhere to the discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements.

Required Qualifications

• Active Speech Language Pathologist license in the state of California, in good standing.
• Demonstrated competency in performing comprehensive patient evaluations and developing evidence-based treatment plans.
• Ability to complete timely and accurate clinical documentation in compliance with state, federal, and payer guidelines.
• Commitment to patient and employee safety standards, including safe patient handling, infection control, and workplace hazard awareness.

Preferred Experience / Skills

• Prior experience in an outpatient therapy setting.
• Experience collaborating within an interdisciplinary care team to facilitate patient progression and discharge planning.
• Active involvement in professional development activities, continuing education, and professional organizations.
