Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM

Key Responsibilities

• Provide physical therapy services to patients in a home health setting within an assigned territory
• Conduct evaluations, develop treatment plans, and deliver skilled PT interventions in the home environment
• Meet productivity expectations of 30 points per week (6 points per day)
• Document patient care accurately and in a timely manner using HCHB (Homecare Homebase)
• Coordinate care with interdisciplinary team members to support patient goals and outcomes

Required Qualifications

• Active Oregon (OR) Physical Therapist license required prior to start
• Previous home health experience required
• Previous experience with HCHB (Homecare Homebase) documentation system required
• Must be able to travel within a 50-mile radius of the office

Preferred Experience / Skills

• Strong time management and self-directed work skills suited for a field-based role
• Experience managing a home health patient caseload independently
