Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Ensure medical record documentation is completed in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
• Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific state practice acts and regulatory requirements; uphold professional standards and codes of conduct

Required Qualifications

• Active Physical Therapist license in good standing in the state of Illinois
• Orthopedic experience required
• Completion of all state-mandated continuing education requirements
• Ability to complete documentation and charge entry using approved formats in compliance with applicable guidelines and policies

Preferred Experience / Skills

• Experience in an outpatient orthopedic setting
• Demonstrated commitment to professional development through continuing education, self-study, and involvement in professional organizations
• Familiarity with performance improvement methodologies and quality of care initiatives
