Key Responsibilities

Evaluate and treat patients across assigned facility locations as part of a split-site coverage arrangement
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward therapy goals
Collaborate with interdisciplinary care teams to ensure quality patient outcomes
Wear a facility-issued photo badge at all times while on site

Required Qualifications

Active Physical Therapist (PT) license in the state of Washington
Valid state or federal issued photo identification required for facility check-in
Ability to work across multiple facility locations as assigned
Availability to work any day of the week, Sunday through Saturday

Preferred Experience / Skills

Prior experience in a skilled nursing or long-term care setting
Experience managing a caseload across multiple sites
