Key Responsibilities

• Perform nursing assessments on all patients within 8 hours of admission and develop individualized plans of care in collaboration with patients, families, and the interdisciplinary team
• Utilize the nursing process — assessing, planning, implementing, and evaluating — to deliver direct patient care with an emphasis on patient and family education
• Provide accurate, timely documentation of all assessments, interventions, and patient responses in the medical record in accordance with policy
• Communicate pertinent changes in patient status to attending physicians, charge nurses, and care team members to support continuity of care and patient safety
• Initiate discharge planning from the day of admission and provide patient teaching in anticipation of the discharge date
• Provide direction, training, and supervision of Licensed Practical Nurses, Certified Nursing Assistants, and orientees, delegating appropriately and assuming charge responsibility as requested

Required Qualifications

• Active Registered Nurse (RN) license in the state of Tennessee or compact eligibility
• Demonstrated ability to perform nursing assessments and develop individualized care plans
• Competency in supervising and directing ancillary nursing personnel including LPNs and CNAs
• Commitment to patient and employee safety practices, including safe patient handling, infection control, and hazard reporting
• Adherence to hospital standards of personal appearance, attendance, and punctuality

Preferred Experience / Skills

• Prior experience in an acute rehabilitation or inpatient hospital setting
• Experience participating in interdisciplinary team conferences and rehabilitation programs
• Familiarity with discharge planning processes and patient/family education best practices
