Contract (13 Weeks) | Monday – Friday, 8:00 AM – 5:00 PM | Onsite/Field-Based

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting across an assigned territory
Evaluate, develop, and implement individualized patient treatment plans to promote functional recovery and independence
Document patient progress and clinical notes using HCHB (HomeCare HomeBase) EMR
Coordinate care with interdisciplinary team members to ensure continuity and quality of patient care
Travel throughout the assigned service area to conduct patient visits

Required Qualifications

Active and unrestricted Physical Therapist (PT) license in the state of Washington
Minimum of 12 months of home health experience within the past 3 years
Proficiency or ability to work with HCHB (HomeCare HomeBase) EMR system
Valid driver's license and reliable transportation for field-based patient visits

Preferred Experience / Skills

Familiarity with Spokane County and surrounding areas
Strong time management and organizational skills suited for an independent, field-based role
Experience coordinating care within a multidisciplinary home health team
