Key Responsibilities

• Conduct skilled home health visits, with an expected 6–8 visits per day
• Perform patient assessments across cardiopulmonary, geriatric, neurological, orthopedic, and oncology populations
• Develop and implement Home Exercise Programs and provide patient and family education
• Utilize and instruct patients on equipment and devices including crutches, gait belts, walkers, sliding boards, collars, and supplemental oxygen delivery systems
• Complete documentation using facility-specific systems including scheduling, timesheet, and forms platforms; OASIS documentation required
• Recognize and report signs and symptoms of physical, sexual, and psychological abuse; adhere to safety and injury prevention protocols

Required Qualifications

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 2 years of experience as a Physical Therapist
• Home health experience required
• Valid driver's license and personal vehicle required to conduct home visits
• Must be COVID-19 vaccinated or have an approved exemption on file; flu shot or declination required — mandatory mask wearing applies in either case
• Locals are welcome — candidates residing within 100 miles of the facility may be accepted at the same rate

Preferred Experience / Skills

• OASIS training preferred
• Cerner EMR experience preferred
• Experience working with cardiopulmonary, geriatric, neurological, orthopedic, and oncology patient populations
