Contract | Days | Monday–Friday, 8:00 AM – 5:00 PM
Key Responsibilities
- Provide physical therapy evaluations and treatments to patients in a home health setting
- Manage a caseload within a 50-mile radius of the office
- Meet productivity expectations of 30 points per week (approximately 6 points per day)
- Document patient care accurately and in a timely manner using HCHB (Homecare Homebase)
Required Qualifications
- Active Oregon (OR) Physical Therapist license required prior to start
- Previous home health experience required
- Previous experience with HCHB (Homecare Homebase) documentation system required
Preferred Experience / Skills
- Strong time management and self-directed work skills suited for a field-based role
- Experience managing a geographically distributed patient caseload
