Key Responsibilities

• Provide physical therapy evaluation and treatment services to patients in the home health setting across Cuyahoga County, northern Summit County, and Lake County
• Develop and implement individualized treatment plans to improve patient mobility, function, and quality of life
• Document patient progress and maintain accurate clinical records in accordance with regulatory and organizational standards
• Collaborate with interdisciplinary care teams to coordinate patient care and ensure continuity of services

Required Qualifications

• Must hold a valid Ohio Physical Therapist license prior to start date — candidates with a pending license are not eligible to begin
• Graduate of an accredited Physical Therapy program
• Ability to travel throughout the assigned service area (Cuyahoga County, northern Summit County, and Lake County)

Preferred Experience / Skills

• Experience with HomeCare HomeBase (HCHB) charting platform
• Prior home health or community-based physical therapy experience
