Key Responsibilities

Provide occupational therapy services to patients in a home health setting across an assigned geographic territory covering Walla Walla County, the lower portion of Columbia County, and into Umatilla County
Collaborate with a manager and interdisciplinary team as part of a geographically assigned care team
Conduct evaluations and develop individualized treatment plans to support patient functional goals
Document patient care and progress using the electronic medical records system

Required Qualifications

Active Washington State OT License required at time of start
Oregon OT License required — candidates may begin with WA licensure while actively obtaining OR licensure
Minimum of 12 months of home health experience within the past 3 years
No on-call requirements

Preferred Experience / Skills

Experience with HCHB (HomeCare HomeBase) EMR system
Familiarity working within multi-county geographic territories
