Key Responsibilities

• Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Complete 60-minute evaluations and treatments, progressing patients toward discharge within an established length of stay
• Maintain accurate, timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation
• Adhere to the applicable State Practice Act and all regulatory requirements, maintaining professional licensure in good standing and completing all mandated continuing education

Required Qualifications

• Active Physical Therapist license in good standing in the state of Ohio
• Completion of all state-mandated continuing education requirements
• Ability to perform in an ethical manner consistent with discipline-specific professional codes of conduct
• Demonstrated competency in clinical reasoning, treatment planning, and medical record documentation

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Familiarity with performance improvement methodologies and quality initiatives
