Description
In this role, you will provide case management services to patients in a fast-paced, high-acuity environment. You will work collaboratively with a triad team of RNs, social workers, and case management assistants. Responsibilities include conducting assessments, coordinating care, and ensuring smooth transitions for patients. The position offers a structured schedule with 8-hour shifts from 8 AM to 4:30 PM, rotating every other weekend.
