Description

Provide skilled nursing, care, and case management for assigned patients in their place of residence. Coordinate and integrate services to ensure continuity of care and facilitate positive clinical outcomes. Oversee care provided by aides and maintain accurate patient records. Participate in team scheduling and quality improvement activities.
